Plan of Care

All care and services furnished by Ability Homecare to care recipients and their families will follow an individualized written plan of care.  The plan of care is a collaboration between the care recipient and his/her family or representative and the care coordinator.  The plan of care acts as a roadmap to ensure that the care and services furnished are consistent and cohesive.
 
The plan of care provides direction for the individualized care of the client.  It is developed from the initial and comprehensive assessment.  The plan of care list all care and services to be furnished by the caregiver and must be developed before services begin.
