Consent for Assistance with Self-administration
of Medications

 

Patients/Clients who are capable of self administering their own medications without assistance are encouraged and allowed to do so.

Ability Homecare has advised me that upon request and informed consent, I may receive assistance with self-administered medication(s) by an unlicensed caregiver(s) referred to me by Ability Homecare.

An unlicensed caregiver means an individual not currently licensed to practice nursing or medicine who is under contract as an independent contractor and/or employee, who has received training with respect to assisting with the self-administration of medication as provided by ACHA Agency rule, to Florida Statutes 400.488.

I hereby request and consent to have assistance with self-administered medication by an unlicensed caregiver referred to me by Ability Homecare.
